McMaster PGME is responsible for maintaining training records of all current and former PGME learners. The PGME office is able to provide confidential composite verifications of training directly to credentialing and licensing bodies.

How to Request

Requesting physicians need to make a written request, and sign an authorization letter to release their information.

Requests for verification and authorization letters can be emailed to postgd@mcmaster.ca

Please indicate if you prefer your request to be emailed, faxed, or mailed to the credentialing or licensing body. Regular mail deliveries can take up to 3 weeks for delivery, depending on the destination.

Replacement certificates can be issued if you have lost, destroyed, or damaged your original certificate, of if you have changed your name.

To request a replacement certificate, please email postgd@mcmaster.ca 

Replacement certificates require the following fees:

Please note that the courier service requires a signature of the receiving party. If it is undeliverable, the courier depot may hold the package for five days before returning it to the PGME Office. It will then be subject to further payment for another delivery.

Please allow three to four weeks for processing.
